Health Screening & Waiver

To comply with Colorado's SB 23-290 Natural Medicine Regulation And Legalization Act, we kindly ask that you complete a health screening form and sign a Release of Liability and Waiver.

Please fill out this short Health Screening Form. This will take you one minute. Your answers will help us understand your needs and provide you with the most supportive and safest experience. We will reach out to you if needed but please note that your answers will likely not affect your participation in the sound bath.

Please also read the Event Details & Release of Liability and Waiver which contains all the information you'll need to prepare for this event. You may return the last page of the waiver with your signature via email to Victoria@InYours.co OR read the waiver in full and give your signature at the check-in desk at the start of the event.
